Blank loot cards?

Do any of you guys have just a blank template for loot cards. I plan on letting my players fill them out themselves as we play, so i would like to have several blank ones. Thanks!

I just hand them an index card to write special abilities of epic loot as they go

I agree with @GmGrizzly, index cards are the easiest option. That being said, what do you think of something like this? Anything you would change or want added/subtracted?

yea this website is way nice!!! although i do wish there was an option to print out a blank one. lol maybe im being too picky.
